I’m 28M, and have been having constant obstructive breathing for the past year. I did several PFT tests which showed irreversible 0.7 FEV1/FVC following ventolin and elevated DLCO (120%).
with my pulmonologist, I did extensive testing to rule out asthma or reactive airways. I also have a clear chest CT with no sign of emphysema.
After trialing many different inhalers my pulmo put me on a trimbow LABA LAMA ICS inhaler.
Before my last PFT, I took the inhaler the morning of the test. The test showed relatively normal values (78% FEV1/FVC with FEV1 = 90% predicted). I also track this with a handheld spirometer at home and only found improvement after taking an LABA LAMA ICS inhaler.
Seeing this result pulmo has now said that they don’t think it’s COPD, we will check for progression in 1 year. But I still have constant obstruction which does not return to normal with a SABA alone.
I believe the reversibility is from the LAMA component, can someone please help me to understand what’s going on, or where to go from here?
Could the obstruction be from mucus production seen in chronic bronchitis before major airway remodelling? Or is significant remodelling typically there at CB onset?
